5 MEDICAL TREATMENT COVER 30000EUR

Assistance Company pays the hospital, operation and treatment expenses and the cost of the medicines prescribed by the relevant physician in case the Insured under this policy becomes ill or injured as a result of an emergency case within the scope of this policy.


6 TRANSFER OF THE INSURED BY MEDICAL REASONS

Insurance Company will coordinate the transfer of the insurance holder in an ambulance or any other vehicle approved by the accompanying doctor or doctor of the Insurance Company to the closest medical centre in case the Insured under this policy becomes ill or injured as a result of an emergency case within the scope of this policy. The relevant expenses incurred during transfer to another medical centre or to the place of residence upon discharge shall also be covered when required. Transfer from one medical centre to another shall only be performed in the absence of the equipment and medical team required for rendering the treatment in the current medical centre. A medical report must be obtained from the attending physician in order to perform the transfer. Only doctors of Insurance Company can decide on the air ambulance service. Countries outside the borders of Europe are not included in the cover provided for air ambulance service.


7 TRANSFER OF THE INSURED TO  HOME

Insurance Holder becomes ill or injured as a result of an emergency case within the scope of this policy and is discharged from the medical centre upon application and completion of the treatment regimen requiring operation, the Company shall pay the expenses incurred for transferring the insurance holder to the Place of Permanent Residence in an ambulance or any other vehicle approved by the accompanying doctor and Company doctor if the doctor of Insurance Company decides he/she cannot continue travelling and original means of transport cannot be used for the return. A medical report must be obtained from the attending physician in order to perform the transfer.


8 TRANSFER OF THE FUNERAL OF THE INSURED

Insurance Company transfers the mortal remains of the insurance holder to the burial place in Turkey. The coverage is exclusive of expenses incurred for the funeral ceremony and burial.


9 LOSS OF LUGGAGE

The cover shall apply provided that a loss, damage or theft report is issued by the relevant airline in case the luggage is lost, damaged or stolen during scheduled flights and it is not found within the period specified by the Airline (minimum 21 days). Insurance Provider  shall make payment up to a limit of EUR 350. The amount of compensation payable by Insurance Provider  shall be determined upon deduction of the amount of compensation paid by the responsible airline and the amount of damages reported by the insurance holder. The insurance holder is obliged to submit a list of luggage contents, estimated price and buying dates and a document showing the compensation received from the airline company. Money, jewellery, credit cards and documents are excluded. Depreciation shall be applied while calculating the value of declared luggage contents that are within the scope of cover. In practice, items bought within a period of six months preceding the date of damage and declared with the invoice shall be paid over the invoice value subject to the cover limit and participation rate. 50% depreciation shall be applied for items that are older than six months (invoice of the relevant item must be presented).



10 REMİTTANCE OF EMERGENCY MESSAGES

Upon request of the insurance holder, Insurance Company shall coordinate the delivery of urgent or necessary messages in connection with the particularities specified above.


11 MEDICAL ADVICE

Insurance Company's medical team provides information on the health problem suffered by the Insurance Holder. All expenses related with this cover shall be borne by the Insurance Holder.


12 PERIOD OF GRACE

A 15-Day period of grace has been included in the cover in addition to the expiry date of the policy coverage in accordance with the European Parliament and Council Directive 810/2009.


13 NOTE

In case any of the aforementioned cover becomes applicable, the amount of compensation limit shall be EUR 30.000 and limit shall be applied in combination in case payment is made under multiple covers which shall be limited with maximum EUR 30.000.




14 SPECIAL CONDITIONS / REMARKS

The number specified at the end of each page should be called before benefiting from assistance services. In case misstatement is made, the policy shall not be deemed valid and no compensation liability shall arise. Services abroad are coordinated by Insurance Company. In case of trips that last longer than 92 days, this duration is not extended without entering the country even if a new policy is issued. Beneficiary within the

scope of the insurance coverage of the travel insurance must be in Turkey when the policy is issued; otherwise, policy shall be deemed invalid. Illnesses (illnesses that continue since the past) that were present before the policy start date are excluded from the cover. Risks arising from all kinds of sports activities and skiing are excluded from the cover. All expenses incurred by airline crew during the flight are excluded from the cover.

Statement: I hereby acknowledge that I have provided accurate and complete information during application for this insurance agreement and I have read and accepted the Special Conditions of the insurance agreement that will be drawn up based on this proposal and General Conditions of Travel Health Insurance and/or General Conditions of Individual Accident Insurance on my behalf and on behalf of dependent persons for whom I submitted the application. I hereby declare, agree and covenant that I authorized representatives of Insurance Provider  An assistance services provider therefore for obtaining information and documents on all aspects from the relevant doctors, healthcare institutions and other relevant parties in connection with the compensation payments (both in Turkey and outside of Turkey).
